License to drill (anywhere)

Staffing shortages are dentistry’s persistent toothache, and no single treatment seems effective. But some states have started prescribing something new: interstate licensing portability. The Dentist and Dental Hygienist (DDH) Compact aims to ease cross-border licensing headaches, making workforce flexibility actually achievable. Consider it ibuprofen for your HR team.
The Morning Grind recently sat down with Andrew Smith, CEO of the Association of Dental Support Organizations (ADSO), who sees the Compact as a vital, practical response to dentistry’s ongoing workforce challenges.
What’s the Compact: Launched in January 2023, the DDH Compact lets member states recognize each other's dental licenses, allowing dentists and hygienists to practice freely across state lines without the costly, slow relicensing process. Smith notes that the compact creates “reciprocity between states,” significantly reducing “... the time, cost, and burden” when transferring licenses.
Nebraska recently became the twelth state to join, following Arkansas, Colorado, Iowa, Ohio, and others. The Compact offers clear benefits to DSOs, enabling them to respond faster to workforce shortages, place providers exactly where needed, and cut administrative costs tied to licensing delays.
Who’s behind it: Support for the Compact spans the entire dental industry—including the ADSO, American Dental Association (ADA), American Dental Hygienists’ Association (ADHA), and even the U.S. Department of Defense (DoD), which recognizes the Compact’s value in easing frequent relocations for military spouses.
Smith emphasizes that the Compact isn’t just a DSO priority; it has backing from “the full profession.” He highlights a generational shift: “The practice of dentistry is changing, particularly with those graduating dental school today, who do not anticipate being in one location for thirty years.” In other words, the Compact matches exactly how new dentists want to practice.
Who’s pushing back: Not everyone’s sold, however. Resistance, Smith notes, has emerged from certain state dental associations concerned about what they say is a loss of control over licensing standards and, perhaps, a potential increase in competition.
Additionally, a competing compact—led by state dental boards—has surfaced directly in reaction to the DDH Compact’s progress. Unlike the DDH Compact, which promotes broad license portability, this alternative compact maintains tighter control, limiting acceptable types of exams and emphasizing state-board-driven standards. Smith characterizes this competing compact as “protectionary,” suggesting it prioritizes board interests over practitioner flexibility.
Bottom line: The DDH Compact is more than regulatory housekeeping—it is organized dentistry's attempt to manage an increasingly mobile workforce, widening dental deserts, and persistent staffing shortages.
Whether license portability fully resolves these issues remains an open question, but in an environment where proven solutions are scarce, the Compact represents a meaningful step toward flexibility. For DSOs, it’s not a cure-all, but it is perhaps yet another signal that tomorrow’s workforce strategy will increasingly rely on agility and adaptability.

Billion-dollar cyberache

Another week, another dental group in the cyber crosshairs. A recent breach saw over 150,000 patient records across the headlines, proving again that DSOs are one of cybercriminals' favorite new targets. As DSOs undergo ongoing scaling and consolidation, the resulting complex IT landscapes and hefty valuations are practically an open invitation for attackers. In light of these increasing threats, DSOs taking proactive measures are setting themselves apart in the eyes of both patients and investors. Turns out that growth hacking works both ways.
What's happening: While recent breaches are grabbing fresh headlines, the cybercriminal love affair with dentistry is nothing new. Dental practices specifically saw over 1.2 million individuals impacted by reported breaches in 2024 alone.
Ransomware remains the weapon of choice, slamming 67% of the healthcare sector in 2024. The FBI has consistently flagged healthcare as the most ransomware-plagued U.S. sector since 2019, noting that dental practices are especially at risk.
The fallout from last year’s Change Healthcare attack, now estimated to affect a jaw-dropping 190 million individuals, continues to ripple through the industry.
Why DSOs: DSOs offer exactly what cybercriminals seek—deep pockets, valuable data, and in some cases, stale IT infrastructure.
A recent Clearwater Security report flagged that hackers view growth and deal announcements not as industry successes but as invitations to exploit integration gaps in patchwork IT systems, betting big on ransomware payoffs. Medical and dental records offer extraordinary black-market returns, further sweetening the deal. In short, the very strategies that accelerate growth also accelerate cyber risk.
The real cost: A successful attack is a crisis. IBM pegs the average healthcare breach cost near $11 million, but the domino effect is worse:
Operational gridlock: Ransomware can shut down practices for weeks, crippling patient care, canceling revenue streams, and grinding operations to a halt.
Regulatory swamp: Breaches trigger HIPAA probes, mandatory patient notifications (miss the 60-day deadline and face serious fines), and corrective action plans. Class-action lawyers pounce within hours of a breach announcement.
Trust torn down: Patient confidence can be significantly reduced after a breach, damaging hard-won brand loyalty.
Hidden bleeding: Staff morale tanks after an attack, leading to turnover. Cyber insurance costs soar.
Security lag: Fast growth in DSOs frequently isn't matched with necessary security upgrades and proactive maintenance, creating a "security lag," akin to technical debt. Security incidents have prompted a shift in security discussions from server rooms to board rooms, with increasing emphasis on the strategic importance of cybersecurity.
As a result, enhanced security makes DSOs not only safer operationally but also more attractive and reliable investment options for PE firms.
Fighting back: Preventing a nightmare scenario isn't just an IT problem; it's a C-suite imperative:
Due diligence is non-negotiable: Buyers, and sellers, would do well to bake rigorous cyber assessments and preparations into every M&A deal.
Unify and standardize: Minimize vulnerabilities by aggressively integrating those disparate IT systems so common in dentistry to create a consistent security posture across the organization. Tackle applying consistent security controls and target weak spots like network segmentation and data protection.
Hunt vulnerabilities: Implement continuous scanning and patching. Annual checkups aren't enough. Add regular penetration testing.
Train your team relentlessly: Your staff is the first defense. Use engaging, ongoing training and phishing simulations to build a human firewall.
Lock down access: Mandate strong, unique passwords and multi-factor authentication (MFA) everywhere possible. Enforce least privilege access.
Bulletproof your backups: Use the 3-2-1 rule (3 copies, 2 media, 1 offsite/isolated), with encryption. And don’t just pray for the best result, test restoration frequently.
Vet your vendors: Scrutinize third-party security rigorously. Require Business Associate Agreements (BAA) and proof of controls.
Practice for disaster: Maintain and test your Incident Response Plan with realistic tabletop exercises. Keep cyber insurance current and know your policy.
Bottom line: Cyber threats aren't slowing down, and DSOs are prime targets. Treating cybersecurity as a strategic pillar—not an afterthought—is crucial for survival and growth. And, increasingly, it is what investors are looking for.
The winners won't just react; they'll invest in resilience, embed security into their scale, operations, and acquisitions playbooks, and make cyber readiness a core competency. In today's threat environment, that’s not just defense—it's how you protect your valuation, maintain patient trust, and secure your future.
